Insulin management can be daunting for physicians of all kinds, as very few have adequate training to optimize management in primary care. Applying naturopathic principles, including First Do No Harm and Find the Cause, helps to identify and illuminate management steps, including when to initiate insulin, proper starting doses, titrating doses, and preventing hypoglycemia and weight gain. This presentation will guide attendees through the confusing terminology and provide guidance that can be immediately implemented in clinical practice to optimize insulin therapy in type 1 and type 2 diabetes. It will present case-based examples of common clinical scenarios to illustrate considerations for starting insulin, dosing long- and short-acting insulin, identifying indications for insulin “supplements”, and preventing complications of insulin treatment. Additional content will include discussions of how to select insulin, how and where to administer insulin, safety issues using insulin concomitantly with other diabetes medications, and considerations when combining insulin with other hormonal and nutritional treatments, including thyroid hormone, adrenal support, and fasting.
